EFFECACY OF ANTIMICROBIAL STEWARDSHIP IN PROPHYLACTIC
ANTIBIOTICS UTILIZATION IN CESAREAN SECTION AT DONG NAI
INTERNATIONAL HOSPITAL
ABSTRACT
Background: Caesarean section increases the risk of infection from 5 - 20 times more
than a vagina. The use of prophylactic antibiotics (PA) has also proven to reduce the risk of
infection in caesarean section and effective as the use of antibiotic antibiotics on selected
patients, which helped save costs and shorten the time of hospitalization . Therefore, the used
of PA in cesarean section was necessary in clinical practice.
Objective: Evaluation of the effectiveness of antimicrobial stewardship on cesarean
section at Dong Nai International Hospital
Methods: Retrospective cohort study on medical records of hospitalized patients from
October 1, 2016 to April 30, 2020, including 2 groups before the intervention (control group,
from October 1, 2016 - March 3, 2017) and after the intervention (intervention group, January
1, 2020 - April 30, 2020). General rationality was assessed based on six factors: justified use
or non-use of PA, antibiotic choice, timing of administration, route of administration, dose,
duration of administration.
Results: We have surveyed on 292 medical records control group and 287 medical
records intervention group, the main age group was 18-35, (85.4%) and the reason for cesarean
section was mainly abnormal because the mother accounts (52.3%). The general rationality of
used PA in cesarean section of control group were 0.0% and intervention group 86.1%
(p<0.001). This intervention had the most positive impact on icreasing the rate of “correct PA
choice” (p<0.001). There was no difference in the effect after discharge in both groups (from
99.7% in the control group and 97.9% in the intervention group, p=0.137).
Conclusion: The program of antimicrobial stewardsship, the overall compliance rate
with reasonable use of PA increased and the post-discharge status was similar in cesarean
section.
