Making a Referral
An employee should not be asked whether they have a disability or any other question about their medical condition.
However, in keeping with the spirit of the Interactive Process, an employee who is struggling to adequately perform
should be asked if there is any type of assistance that might enable the employee to better perform their job functions
and, along with other material describing employee support resources, the employee should be given information about
company policies/procedures applicable to employees with disabilities. Please have the employee complete and submit
an Americans with Disabilities Act (ADA) Accommodation Request form and submit to the HR Department.
Risk & Expectation
Employees have the right to report work-related injuries and illnesses. If an employee is injured on the job, the employee
should immediately notify their manager.
The manager’s immediate responsibility is to send the employee for necessary medical treatment. The manager is also
responsible for conducting an incident investigation and documenting all facts related to the incident and any injury
sustained immediately. The manager should then contact HR.
