Choose one of the patients from the previous lessons. Fill in the
insurance form with information about that patient.

E. Culture tip

English has many national and regional varieties. Medical insurance forms from Britain
will use British English; those from America will use American English. The Thai doctor
writing a report should not be overly concerned about such matters. Accuracy and
completeness of the information supplied is much more important than grammatical
accuracy or spelling and adherence to appropriate regional varieties of English. Besides,
there is an excellent chance that the reader of the report or form will not be a native speaker
of English him-/herself.
