WELCOME TO YOUR BENEFITS!

Mission Pharmacal is pleased to provide you with the information you’ll
need to enroll for next year’s benefits. We believe this enrollment guide will
make it easier for you to learn about your benefit plan options, decide on
levels of coverage that are right for you and your family, and compare costs
before you log into ADP website to complete your online enrollment for
2022 benefits.

Be sure to consider what your needs may be for the upcoming year and
choose your benefits accordingly.

OPEN ENROLLMENT
Begins: November 15, 2021 (12:01 CST)
Ends: November 29, 2021 (11:00 PM CST)

This year will be a PASSIVE ENROLLMENT, so your previous benefit elections
will roll into the new plan year if you do not make changes or decline coverage.
However, employees must make ACTIVE ENROLLMENT elections for Flexible
Spending Accounts (“FSA”) for the 2022 Plan Year to continue coverage. If you
do not make FSA elections, your 2021 benefits WILL NOT ROLL OVER; your
FSA benefits will terminate on December 31, 2021.

Participants of Flexible Spending will be required to sign-up for direct deposit
directly through BPAS.
