DRUG ADDICTED PATIENT CARE SYSTEM WITH SEAMLESS DEVELOPMENT FROM HOSPITAL TO COMMUNITY

Abstract
Due to increasing of drug addiction problems in Thailand. In addition, the increase number of drug addicted patients in the hospital. The patients were treated from hospital and had to go back to community and family. Thus, this project was established to develop a drug addicted patient care system with seamless from hospital to community and to evaluate the results of development 2 communities and hospitals were selected for this project, which were the community and hospital of Amphur Munjakhiri and Amphur Nampong. Both community and hospital participated by drug addicted patients, who were serious mental illness with high risk to violence ( SMI-V) , members of family, community health volunteers, staffs of the sub-district health promotion hospital, Nurse staffs of the hospital, and the law regulator in the community. The action of development used in the project were following Kemmis and McTaggart principle, which uses 4 steps of planning, action, observation, and reflection. Then, the project was evaluated of drug addicted patient care. The project started from 2021 to 2023 for 3 years. The results showed that the committee of the project was established and did planning patient referral system between hospital and community in dimension of data sharing, communication, and care taking with seamless activities of patient care including monitoring of observation and reflection of taking care drug addicted patients continuously. After doing the program as planned. The evaluated result showed that the patients were recruited in the program for 495 cases. 495 cases (100.0%) were taken care all steps in taking care system by the hospital and community continuously using coworking of hospital and community with seamless development from hospital to community. 483 cases (97.3%) were in better status (being taken care all standard steps, no aggressive behaviors, and compliance in drug addicted patient care) So, the Coworking of community and hospital was important to take care drug addicted patient and could share together of planning, action, observation, and reflection for taking care of patient with seamless.
