Confidentiality -Vital For Patient Privacy
1. It is the LAW -FCS has a legal and ethical responsibility to safeguard the
privacy of all patient information.
2. As a volunteer, you may come in possession of confidential information. You are only to
access and use this information when it is necessary to perform your volunteer service.
3. You are not to discuss confidential information where others can overhear the conversation.
It is not acceptable to discuss confidential information, even if the patient's
name is not used.
4. You are not to disclose or discuss confidential information with others, including friends or
family who do not have a need to know that information.
5. You are not permitted to use any FCS computer or information technology
systems or devices.
6. You are required to sign and date the FCS Foundation Confidentiality Agreement and
comply with all terms and conditions included in this form.
7. By signing this agreement, you understand that violation of the Confidentiality Agreement
may result in disciplinary action, including suspension and/or loss of privileges, up to
termination of your volunteer services.
