Sample Office Financial Policy
For Patients Without Dental Insurance
Payment is expected in full at each visit as treatment progresses with:
1. Cash
2. Check
3. MasterCard / Visa / American Express
4. Care Credit
5. Prepaid Dental Plan
For laboratory cases ( crowns,bridges,dentures,partials) ½ down is expected at the first
appointment and the balance upon completion of treatment.
For Patients With Dental Insurance
Payment of the deductible each year on the first visit and out of pocket portion as treatment
progresses.
