IDENTIFY, SELECT AND CHECK SAFETY EQUIPMENT FOR
SERVICEABILITY
Working at heights can be a dangerous task, whilst you are working it is important that
you identify, select and check all safety equipment for serviceability. This is important to
ensure that your safety systems are working properly and will keep those working with
and around you as safe as possible.
Safety equipment must be inspected for defects and issues reported and rectified. The
health and safety requirements, policies and procedures will outline the reporting
procedures at your workplace.
A safety checklist should be used to conduct inspections of the equipment. Records of
inspection, faults and the rectifications to be completed should be documented and filed
in an accessible location for future reference.

Access Equipment Inspection Checklist
1. Entry and exit
Is there safe entry and exit to every scaffold platform?
Are temporary stairways correctly installed?
Are portable ladders of an industrial grade, serviceable and correctly installed?
Are entries, exits, and access platforms correctly installed?
Are ladders securely fixed to prevent movement?
Are ladders pitched at a gradient not less than 1 in 4 nor more than 1 in 6?
Do ladders extend at least 900 mm above the platform?
