Flex Benefit Debit Card

A Flex Benefit Debit Card is available at no cost to the employee.

The Flex Benefit Debit Card is a convenient way to access your FSA funds at the point of sale rather than
submitting a paper claim and waiting for reimbursement. You can use it to pay for office visit and prescription
co-pays, health insurance deductibles and qualified over-the-counter expenses.

            • The Flex Benefit Debit Card draws funds directly from your current FSA account.

            • The card should only be used for expenses that you and your eligible dependents incur within this plan
year. It cannot be used to pay for services incurred in a prior plan year that are been being billed for in
               this plan year.

            • A service must be rendered before payment can be made with the Benefit Card.

            • Do not throw out any receipts! Even if your transaction goes through at the register at time of
purchase, you still may need to provide documentation.

        Shop FSA – Health Shopper

When you log in to your Flexible Spending account at www.csone.com and click on “Shop FSA,” you will be
taken to a site called “Health Shopper.” Health Shopper works along with Amazon to allow you to shop for
flexible spending qualified items and have them shipped directly to you. Only flexible spending qualified items
will be on the Health Shopper site. By logging in to your csONE Flexible Spending account and clicking on
“Shop FSA”, you will be connecting your Flex Benefit Debit Card to the Health Shopper site and the
corresponding Amazon checkout. You will choose your items from Health Shopper and they will go into an
Amazon cart. At checkout, your Flex Benefit Debit Card will be used to pay for these items. Amazon will then
have your flexible spending qualified purchases from the Health Shopper site delivered to your home.

In the ever changing and challenging environment we’ve come from, we at NHS know how difficult it can be to
juggle the health and well-being of our families and our own selves. We want to remind staff to continue self-
care and encourage whole being wellness, mindfulness, care and safety. We will be working hard throughout
the year to provide interesting offerings to help us continue on this path and to remind us that there are many
aspects to whole-being health. We want staff to be encouraged and support each other to participate in
challenges, sharing creative idea and success stories!
