i. If you witness a third-party causing damage or you cause any damage to any property
on or off site, no matter how small, you must write down full details of the damage and
how it was caused on your insurance form. If you cause damage, you must call your
Manager to report the incident and the form must be given in to your Manager as soon
as you next return back to your operating yard or head office.
10.1 Insurance Details
Notification of any accident, however slight, must be made to the Transport Team
immediately who will pass on the details to the Transport Manager and Directors.

• The Company’s name as the vehicle owner and the address.
If an accident has occurred, you must complete a statement of the facts
straight away on the form in section 10.2 together with a brief diagram
showing the point of impact of both vehicles. If you are able to use a digital
camera to gather additional evidence if safe to do so this would be very
useful in support of any claim.
10.2 Road Accident Form Example
Once completed, this form must be handed into your manager as soon as possible and a
Motor Accident Report form completed by the driver with the assistance of your line manager
