The following graph shows the breakdown of systems issues
in the surgical specialties. The majority of these issues involve
wrong-site/procedure incidents or retained foreign body. The
risk issues found in the physician’s office include: failure to
track, failure to notify patients of abnormal results, and failure
to follow-up on patients that were no-shows for follow-up
appointments. We will discuss wrong-site/procedure incidents
and retained foreign body later in this course.
