Discharge Instructions
Discharge from the hospital is a vulnerable time for patients.
Clear and easily understood instructions are imperative for
enabling a smooth transition in the responsibility of care as it
shifts from providers to patients. Miscommunications at the
point of discharge are common, particularly when there have
been medication changes and required follow-up. It is
important to be aware of state and hospital rules, regulations,
or opinions that may prohibit delegating this duty.
