COLLABORATIVE AGREEMENT / PROTOCOLS

☐ Ensure jointly-developed protocols outline criteria
describing appropriate consultation with the supervising
physician or referral to a specialist; and how to contact the

physician and backup physician.

☐ Jointly outline the standard of care and scope of practice
to ensure services are lawful and within the APP’s training
and competency.

LICENSURE & VERIFICATION

☐ Obtain any prescreening background checks required by
the state (Department of Health or board websites often
post links), always check references, verify licensure, and

document findings.

☐ Post any required notices for title transparency, Adult
Protective Services, domestic abuse hotline, anti-

discrimination, etc.

☐ Ensure maintenance of licensure and necessary
credentialing.

☐ Update protocols, formularies, and other state specific
requirements. Review hospital and facility bylaws to ensure
compliance.

REGULATORY COMPLIANCE

☐ Provide training and ensure compliance with HIPAA, the
Health Information Technology for Economic and Clinical
Health Act (HITECH), as well as any relevant state privacy

laws
