Documentation Advice Dr. G. Randy Smith has
-Dr. G. Randy Smith some practical advice for
hospitalists on the subject
of documentation and, with

When thinking about
malpractice risk and
3
documentation, the key thing
to remember is context, G.
Randy Smith, Jr., MD, MS,
4 told attendees at the 2019
Midwest Hospital Medicine
Conference, held in Chicago
5 in October 2019.
As the care for hospitalized
6 patients is initially being
documented, “Everybody’s
living in real time with us -
7 the nurses, our consultants,
the patient - and everybody
8 involved kind of understands
the context of the moment,”
said Dr. Smith, an assistant
9
professor of medicine and a
hospitalist at Northwestern
University in Chicago who
10 has served as an expert
witness in lawsuits.
