abbreviated as “ePHI”. Individually identifiable health information

includes demographic data that relates to:

 The individual’s past, present or future physical or mental

health or condition;

 The provision of health care to the individual; or

 The past, present, or future payment for the provision of

health care to the individual; and

 Identifying the individual or for which there is a reasonable
basis to believe it can be used to identify the individual.

Individually identifiable health information has many common
identifiers which include those listed on following page.

To be considered “de-identified”, all of the 18 HIPAA identifiers must

be removed from the data set. This includes all dates, such as

surgery dates, all voice recordings and all photographic images.
