Fields with an asterisk are required

 Make sure to enter the compliance information in the specific fields: Certification Code, Certification Date, Indicator, Approval
Date, Assurance Number, Exemption Number (only if indicator is exempt), and Expiration Date. This is the minimum required
information for a certification.
 Please make sure to attach all documentation available at this stage. Having supporting documentation can potentially
shorten the review lifecycle and ensure that the generated award requires limited changes.
