          SERVICE & ENROLLMENT


                                           Summary of Services for
                         Retiree Healthcare Programs and Administration



          While this comprehensive list of services is not intended to be all-inclusive, it should provide greater
          understanding of the core functions used to support your retiree program.  These services are
          provided at initial enrollment and as part of the ongoing administration of your program and are
          included in the rates presented.  The services contemplated and described herein are for all eligible
          Post-65 retirees.

          Plan Design and Development


              1.  Identify your retiree program needs to improve upon your overall retiree healthcare program.
              2.  Develop plans as requested and offer alternatives that may fit your needs.
              3.  Develop underwriting approach to disseminate to insurance carriers for comprehensive plans
                 and competitive quotes.
              4.  Receive rate quotes and corresponding business rules.
              5.  Present proposed benefit options, rates and carrier business rules.

          Documentation Review and Preparation

              6.  Draft and assemble program documentation.
              7.  Legal/contract review, as needed.
              8.  Final documents to all parties for signature.
              9.  Obtain business rules from all providers and disseminate to internal parties.
              10. CMS documentation as necessary.


          Program Implementation

              11. Identify implementation team and assign responsibilities.
              12. Develop implementation tasks and timelines.
              13. Client implementation review, acceptance and sign-off.
              14. Marketing material development.
              15. Employer Branding developed and used for program communications
              16. Client communication letter(s).
              17. Enrollment kit materials, design and client sign-off.
              18. Fulfillment kit materials, design and client sign-off (welcome letter and kit contents)
              19. Enrollment materials for newly retired and “soon to be” 65 eligible members.
