Week 13 for CFC General
• 2nd Meeting with parents in November. To discuss steps taken with child since last meeting in week
6.
• This will allow the majority of children with CFC behaviours to be removed from the list due to
significant development.
• • Those children still requiring CFC support, new Observation Forms and Action Plans in place every
two weeks.
• Forms 2, 3 and 4 completed again.
NB: Observation are to be recorded for two weeks then an Action Plan will be created using that
information. This is an ongoing process every two weeks.
Week 17 onwards
• EYVP will create a list for potential children to be blocked.
• • EYVP will check all files to make sure documents are uploaded to the drive and completed correctly.
• EYVP will carry out one final observation to approve or reject block request.
Week 22
• Invitation to be sent to parents for 3rd meeting
Week 22 for CFC General
• 3rd Meeting with parents in January/February.
• • At this meeting the parents will be required to sign that their child will be BLOCKED for the
following year at all EPG facilities. Complete Form 6 (Non-reenrollment).
• These children will follow their Action Plans until the end of year and continue to meet with the
parents.

Any child who shows significant progress during this time may be removed from the BLOCKED list. The
concerns will still be recorded on the FS2 Profile.
