• Treat all participants, equipment, and property with care

• Refrain from disruptive, aggressive, or inappropriate behavior

4. Responsibility for Damages

Talent and Guardian acknowledge that:

• Talent is responsible for any damage caused to equipment, props, costumes, or
facilities due to negligence or misconduct

• Guardian agrees to reimburse Media Group for any repair or replacement costs
resulting from such damage

• Media Group reserves the right to remove Talent from future participation if
damage is repeated or intentional

5. Allergy & Medical Disclosure

Please list any known allergies, sensitivities, or medical conditions that may affect
participation:

Guardian agrees to provide any necessary medications, instructions, or emergency
contacts related to the above.

6. Off-Location Recording Clause

Talent may be invited to participate in off-location recordings, including outdoor shoots,
studio visits, or travel-based projects. Guardian agrees to:

• Ensure Talent is prepared and dressed appropriately

• Communicate any location-specific concerns in advance

• Assume responsibility for transportation unless otherwise arranged
