Creative Services Consent Form
(Makeup Artist, Stylist, Photographer, Videographer, Content Creator, Acting Coach, Trainer)

This Creative Services Consent Form (“Form”) is entered into by the undersigned
parent or legal guardian (“Guardian”) of the minor child (“Talent”) named below, in favor
of Baby Kid Teen Media Group (“Media Group”).

1. Consent to Participate

I authorize my child to receive services from approved creative professionals affiliated
with Baby Kid Teen Media Group and/or its clients. These professionals may include:

 Makeup artists and hair stylists
 Wardrobe stylists and costume designers
 Photographers and videographers
 Content creators and digital editors
 Acting coaches and performance trainers

These services may be provided during auditions, rehearsals, showcases, interviews,
photoshoots, video recordings, and related creative activities.

2. Service Locations & Setup Requirements

With prior written permission, Media Group may schedule services to be performed in
one of the following approved environments:

 The professional’s home studio or workspace
 An on-location site selected for creative or logistical purposes
 A third-party set or venue approved by Media Group

If services are performed in the family’s home, I understand and agree that:

 The home must be clean, safe, and free of hazards
 Animals must be secured or put away during service hours
 A designated area must be available for setup, including space for backdrops,
lighting, and equipment
 The homeowner must grant permission for services to be performed on-site
 All professionals are vetted, bonded, and trained to work respectfully and
safely in family environments
