This Booking & Scheduling Consent Form (“Consent”) is entered into by the
undersigned parent or legal guardian (“Guardian”) of the minor child (“Talent”) named
below, in favor of Baby Kid Teen Media Group (“Media Group”).

1. Consent to Schedule & Coordinate

I authorize Baby Kid Teen Media Group to coordinate and schedule my child’s
participation in auditions, rehearsals, showcases, interviews, and related activities. This
includes the right to assist with travel arrangements when necessary.

2. Travel Coordination & Booking

With prior written permission, Media Group may book the following on behalf of Talent
and Guardian:

• Hotel accommodations

• Flights and ground transportation

• Valet services or shuttle arrangements

• Event-related lodging or travel packages

Media Group will share all booking details and costs in advance. Guardian agrees to
pay all approved fees directly or reimburse Media Group as agreed upon.

3. Corporate Discounts & Rate Matching

Media Group may access corporate discounts for travel and lodging. If Guardian can
secure a better rate, they may use their own booking—but must:

• Share full booking details with Media Group

• Submit confirmation at least 7 days prior to the scheduled event

• Ensure bookings align with Media Group’s schedule and logistics
