Accident Waiver & Release of Liability Form

This Accident Waiver & Release of Liability Form (“Waiver”) is entered into by the
undersigned parent or legal guardian (“Guardian”) of the minor child (“Talent”) named
below, in favor of Baby Kid Teen Media Group (“Media Group”).

1. Assumption of Risk

I understand that my child’s participation in Media Group activities—including auditions,
rehearsals, showcases, interviews, photoshoots, video recordings, and off-location
filming—may involve physical movement, travel, equipment, and interaction with other
participants. I voluntarily assume all risks associated with these activities.

2. Release of Liability

I hereby release and hold harmless Baby Kid Teen Media Group, its staff, volunteers,
affiliates, and partners from any and all liability, claims, or damages arising from:

• Accidents, injuries, or illnesses sustained during participation

• Travel to and from scheduled events or off-location recordings

• Use of props, costumes, makeup, or equipment

• Any unforeseen incidents occurring on set or during Media Group activities

This release applies whether caused by negligence or otherwise and is intended to be
as broad and inclusive as permitted by law.

3. On-Set Professionalism & Conduct

Talent and Guardian agree to:

• Maintain respectful, professional behavior at all times

• Follow instructions from Media Group staff and crew
