               National State Talent Compliance Form

               This State Talent Compliance Form (“Form”) is entered into by the undersigned parent or legal
               guardian (“Guardian”) of the minor child (“Talent”) named below, in favor of Baby Kid Teen
               Media Group (“Media Group”).

               1. Purpose of This Form


               This form ensures that all legal, financial, and regulatory responsibilities related to your child’s
               participation in entertainment projects are clearly understood and properly managed. Baby Kid
               Teen Media Group is not a talent agency or modeling agency, but a nationally recognized
               media company that works with top brands and networks to book talent and produce impactful
               campaigns.




               2. Parent/Guardian Responsibility


               Guardian agrees to assume full responsibility for:

                     Obtaining and maintaining all required state work permits, performance licenses, and
                       labor documentation
                     Opening and managing any required Coogan Trust Account or equivalent minor trust

                     Providing documentation to Media Group and/or clients upon request
                     Ensuring compliance with local, state, and federal laws regarding child performers

               Media Group will assist with coordination, provide guidance, and help facilitate communication
               with clients and production teams, but final responsibility rests with the Guardian.




               3. Coogan Account Requirements


               In accordance with laws in states such as California, New York, Illinois, Louisiana, New
               Mexico, and others:
