6. Patient must demonstrate Rehabilitation potential, i.e., the ability to benefit from a
Rehabilitation program provided within the home setting.
MANAGED CARE
1. Prior approval is needed before any visits are made. The system will not allow any
appointments to be scheduled or visits made if there is no authorization.
2. Managed Care requests will be done by the office based on your documentation.
3. An accurate and complete visit note is instrumental to generate a thorough request.
4. You can view the insurance information and any authorized visits in the Medical Records,
under demographics, and payer source.
