Start of Care OASIS
The SOC OASIS is an assessment tool completed at the beginning of the initial episode of
care. The purpose is to:
1. Define a HHRG score based on the answers to the OASIS questions which will dictate the
reimbursement rate;
2. Gather information that will define outcomes of the home care services provided.
HHRG – Home Health Resource Group
1. Responses to specific OASIS questions are organized into 3 domains which produce the
HHRG score:
f. Clinical Severity:
i. Affected mostly by home care diagnosis and wound items
g. Functional Status:
i. Affected by patient’s level of assist for:
1. Bathing
2. Dressing
3. Toileting
4. Ambulation
5. Transfers
h. Service Utilization:
i. Episode Timing- what episode of care are they in i.e. 1 , 2 , 3 etc.

ii. Therapy need – number of PT, OT, ST visits needed
2. Each question has a point value based on the severity.
3. Scores are totaled within each of the 3 respective domains and assigned to severity categories
from 1 to 4 (with 1 being least severe to 4 most severe)
4. The combination of the 3 totals creates a Home Health Resource Group (HHRG)
PPS has payment variations
3. LUPA: Low Utilization Payment Adjustment- Four visits or less are paid on a per visit basis.
4. PEP: Partial Episode Payment- patient is admitted to Agency A, then elects to transfer to
Agency B. Agency A receives a proportional payment based on length of stay.
Relationship between OASIS information and the Plan of Care (POC)

4. Accuracy is critical - prepare ahead of time; know what to assess
The SOC OASIS questions are scattered all over the entire PT/ST00 visit. Most of them are in the
Physical Assessment section of the visit note. The SOC visit note is similar to an Add-On
Evaluation note with similar sections, e.g. mileage/drive time, vital signs & measures, pathways, etc.
but with more required items, indicated by red asterisks.
