If the patient is unable to sign and there is no caregiver present, select “caregiver” and you will
have a text box to explain the circumstances (e.g. Patient unable to sign d/t cognitive deficits,
daughter unavailable at this time, HHA not allowed to sign. Verbal consent obtained from
daughter to continue visit)

**NOTE: Once Electronic Forms is checked off, DO NOT go back into this section. The system
will recognize this action as you editing this section and will require the patient to sign again.

Demographics

Unlike other visit service codes (PT/OT/ST 01 or 11s), the Demographics section of the 00 visit is
a required section, with several required subsections (as indicated by red asterisks). Complete all
required items in this section.

*Note: This section does not indicate whether or not you completed a subsection. There
will be no green check marks, etc. Keep track of your progress. If you click on Save and you
are still missing required items, the system will indicate them.
