Entitlement
This is a required item in the SOC that is not in the Add-On eval. This section contains questions
regarding the patient’s Medicare entitlement, and if he has other entitilements (e.g. Black Lung
Benefits, Workman’s Compensation, etc.)
Vital Signs & Measures
This section’s requirements are similar to any visit. However, for a SOC visit, the clinician is
required to view the Parameters.
Pathways on a SOC visit
Similar to the Add-On Eval visit, the clinician sets the rehab plan of care in the Pathways. However,
since this is a SOC, the clinician performing the SOC must create additional pathways in addition to
their discipline’s pathways, and pathways for other disciplines if needed.
For example, if the patient has a wound, the pathway for Altered Skin Integrity must be selected,
with a sub choice of “Need for O/A of skin” (observation and assessment). Or if there are
additional disciplines needed and there are no existing orders for them, “Need for Other
Disciplines” must be selected.
