Provider Care Services
Home Visiting Physicians
Our Home Visiting Physicians program is a collaboration
between VNSNY nurses and the interdisciplinary team
at Mount Sinai Medical Center. Together, we provide
high-quality care and social support to the homebound
population throughout Manhattan.
Program elements include joint home visits from a Mount
Sinai physician and our nurses, as well as clinical rounds on
a routine basis, to ensure that a comprehensive plan of care
is in place to promote the health and well-being of patients
and their families. The goals of this program are to support
evidence-based clinical practice in the home, decrease
avoidable hospitalization rates and promote expert,
inter-professional teamwork in the delivery of services.
The Home Visiting Physicians program also includes a
collaboration with the Mobile Acute Care Team (MACT) of
Mount Sinai to facilitate Sub Acute Rehab (SAR) services.
