Provider Care Services



        Social Work Services


        Social workers play a vital role as
        part of the interdisciplinary care
        team.  Social work services focus
        on the current medical condition
        and the social determinants of
        health and well-being. The social
        worker conducts an assessment
        to determine patient needs and
        identify any barriers that might
        impede recovery. The goals of care include:

        •   Helping the patient and caregiver to adjust to and cope
          with the current medical condition
        •   Improving treatment adherence
        •   Avoiding re-hospitalization
        •   Strengthening the patient’s support system through

          linkage to appropriate community resources, services
          and public benefits
        •   Ensuring that the patient’s basic needs are met in a safe
          home environment
        •   Assisting with the development of long-term care plans

        Social work services are provided only in conjunction with
        skilled nursing or rehabilitation services.
        Service Area:  All five boroughs of New York City, Nassau,
        Suffolk and Westchester Counties.
