        Other CMHS Programs


        Assertive Community Treatment (ACT) is for adults who
        are seriously mentally ill living in the Bronx, Queens and
        Manhattan, and who have a history of long-term psychiatric
        hospitalizations. For information, call 718-742-7114 for the

        Manhattan.

        Comprehensive Care Management (CCM) provides
        case management and vocational job preparation services
        to adults living in Manhattan, Queens and Staten Island,
        who have substance abuse issues and who receive public

        Home-Based Crisis Intervention (HBCI) provides short-
        term family-centered services for families with children
        who are at imminent risk of hospitalization due to a serious
        psychiatric crisis and who live in the Bronx and Brooklyn.

        Promise Zone is a Bronx-based, free, voluntary school-based
        mental health preventative program that operates over
        a period of 6-8 weeks, in which an MSW/LMSW actively
        meets with a child ages 5-17, on average one time weekly,
        to identify and target disruptive psychological symptoms
        being reported by guardians and/or schools. For more
