Patient’s Assessment and
Communicating Nurse Care Plan
Case Scenario
A 70-year-old male, admitted two days ago for a hip fracture surgery, reports experiencing severe pain
and limited mobility in his operated hip during the morning rounds. He also mentions feeling dizzy and
having a slight increase in temperature. The nurse will perform a comprehensive assessment and
communicate the nursing care plan to the patient.
Nursing Diagnosis:
● Acute pain related to physical injury agent.
● Risk of infection related to surgical wound.
Nurse's Plan:
● Pain management: To alleviate pain and enhance mobility
● Infection control: To monitor and prevent potential infection
Task: Perform a dialogue between nurse and patient in the English while the nurse conducts the
assessment and communicates the nursing care plan.
