                       Hydration & Nutrition Administration




                 Case Scenario


               A male patient. 45 years old, was admitted to the hospital with complaints of severe vomiting, diarrhoea,
               and abdominal pain for the past two days.  The patient reports that the pain is crampy and located in the
               lower abdomen. He mentions having multiple episodes of watery diarrhea, which he estimates to be
               about 6-8 times since yesterday. He also mentioned having severe vomiting about 10 times yesterday. He
               also noted not being able to keep down food or medicines because of the severe vomiting. The patient
               had no significant past medical history. The physical assessment showed dry lips, sunken eyes, bad skin
               turgor,  abdominal  tenderness  in  all  quadrants,  and  more  pronounced  in  the  lower  abdomen,  bowel
               sounds  are  hyperactive.  No  rebound  tenderness  or  guarding  was observed.  Vital  signs  assessment

               showed: Temp: 38 C, HR: 102 bpm, BP: 130/90 mmHg, RR: 28X/minute. The medical team diagnosed the
               patient with severe gastroenteritis with dehydration and inability to take oral medications or fluids due
               to persistent vomiting. His condition led to dehydration, electrolyte imbalance, and the inability to keep
               down food or medications. After an assessment by the medical team, it was decided that he requires both
               intravenous fluid therapy to correct the dehydration and an NG tube to manage his nutritional needs,
               administer medications, and prevent further vomiting.

               Nursing Diagnosis:
                   •  Acute abdominal pain related to gastrointestinal disturbance.
                   •  Fluid volume deficit, with diarrhoea risk factors and severe vomiting.
                   •  Risk for nutrition deficit with diarrhoea risk factors and severe vomiting

               Nurse's Plan:
                   •  Intravenous Catheter (IV) Placement: To administer fluids and medications.
                   •  Nasogastric (NG) Tube Placement: To relieve vomiting and administer nutrition and medication,
                       as the patient cannot tolerate anything by mouth.

               Task: Perform a dialogue between nurse and patient in the English language when the nurse performing
               the IV and NGT placements
