Discharge Planning and Health Education
Case Scenario
A 32-year-old male patient was hospitalized after undergoing surgery on his left hand. The patient
experienced a work accident that caused a deep laceration. To close the wound, the doctor performed a
skin graft procedure. Today is the third day of treatment, and the patient is allowed to go home after
their condition is deemed stable.
Before the patient leaves the hospital, the nurse conducts discharge planning by providing general
information about the patient's current condition, the purpose of the medical procedures that have
been performed, home care, and the follow-up plan. The nurse also educates the patient on home care,
signs of infection, activity control, diet and nutrition, as well as pain and medication management. At
the end of the health education session, the patient leaves the hospital with a copy of the medication
prescription, a control schedule sheet, and a care guide.
Nursing Diagnosis:
• Deficit knowledge about home care procedures is related to a lack of exposure to information.

- activity pattern management
- dietary management
- pain management and medications
