Elimination Assistance
Case Scenario
A woman, 35 years old, was admitted to the surgical ward, planned for orthopedic surgery due to a fracture
of the tibia fibula of the right leg. The patient's condition is fully conscious but all activities are carried out
in bed, complain pain when moves his legs. When the nurse carried out an examination during the day, the
patient complained that he wanted to continue praying but was worried that his clothes would be unclean
because he previously used a bedpan for urinating and his urine was a little spattered. The nurse then
planned to install a urinary catheter to make the patient more comfortable and in preparation for the surgical
procedure. The patient received several drug therapies, one of which was stool softener because he had not
defecated for 3 days. In the afternoon the patient's family activated the bell to ask the nurse for help because
the patient felt like defecating. The nurse then prepares diapers equipment to help the patient defecate
because it is more possible and easier according to the patient's condition.
Nursing Diagnosis:
• Readiness for increased elimination of urine
• Risk of constipation
Nurse's Plan:
• Urinary Catheter Placement: To support urinary process
• Diapers Removal and Placement: To support defacation process.
