Underline the correct verb to complete the nurses’ entry in a journal on psychiatric ward.
I try to establish a therapeutic relationship with a 9 year old who developed PTSD following an
accident. Hehas been drawing/has drawn planes that were on fire and crashing, unable to sleep alone,
unable to eat, unable to separate from his family, and unable to leave his home. I have talked/have been
talking to her, I think the problem is that she feels she have been/has been neglected. When I said to her,
“How long have you drawn/have you been drawing here?” at first she refused me. “You look very
uncomfortable”, I said. “You have not been eating/have not eaten and you must have been/have been
being very hungry.” She suddenly grasp my hand and ask me to take her to the kitchen. She said, “You
haven’t been walking/walked away from me”. “I can’t walk away until I have been getting/got you a
little morw comfortable”, I said. She smile and for the rest of my shift she becomes more cheerfull.

Journal Entry
Monday 08.00 am We had a patient admitted to the unit who was quite unkempt, appeared very
grumpy, was very quiet and kept to herself. Some staff have dismissed this
behavior as hostile and resistant to treatment. I see something different. I see a
woman who is struggling with her marriage, struggling with parenting,
struggling with finances and most of all struggling with depression and horrible
self-esteem.
SYMPTOMS
11.00 am I make it a point to sit by her during meals and introduce conversation with
her. She is very guarded because she have been hurt so many times in the past.
We don’t talk much at the beginning, but gradually she opens up to me and she
starts to blossom. From our conversation, I have learned all about her kids and
how smart she was. She had awesome goals for herself, but never feel
confident enough to pursue them
