Now the patient has calmed down. After a few days, the patient starts to get better. He
begins to talk to other staff. He still sometimes hallucinates about the man, but as the
treatment is running, he starts to be able to control it. The staff is thankful and amazed. They
have learned a valuable lesson that talking with the patient and meeting his needs, even if it
is unclear to us, may be a simple answer to solving an acute issue.
I try to talk to him but he always refuses me. He has been screaming all day and start to
bother the other patients. So, I try to talk to him again, I say that I can talk to the man also.
After saying that, he become calmer. I tell him that I can also see the man. I try to assess him
by asking him what he is worried about.
I am working on a med-tele floor and ask to assist with a psychotic patient who cannot be
managed. He always hallucinates about a man watching him. According to staff they have
tried everything including all available medications. They have restrained the patient and
nothing have worked. I approach the patient and he is screaming, terrified of the man in the
corner of his room who is there to kill him.
After talking with him, I come up with a conclusion that the only thing he is afraid of is the
man is his hallucination. So, I ask him if he want me to ask the man to leave and he says yes.
I tell the man firmly to leave and ask him not to come back. I ask the patient if he want me
to stay and held his hand to make sure he is safe and he agrees. I do and the patient fall
asleep.
