In short, nursing interventions column is really two parts:
• First, what nurses will do to help the patient reach their goals.

What is an evaluation column?

It is fantastic if they are able to meet the goals. Thus, you do not need to do anything else.

Vocabulary: Action verbs to describe the interventions
List of action verbs you might use in your nursing intervention:
• To monitor : to watch, keep track of, or check usually for a special purpose
• To review : to view or see again
• To assess : to determine the importance, size, or value of
• To record : to set down in writing, : furnish written evidence of
• To encourage : to inspire with courage, spirit, or hope
• To suggest : to propose as desirable or fitting
• To measure : to discover the exact size or amount of something
• To elevate : to lift up or make higher
• To ensure : to make sure, certain, or safe
• To document : to record the details of an event, a process, etc.
Examples of the application of the action verbs in the interventions
➢ Assess abdomen and GI status: bowel sounds and distention.
➢ Monitor the client’s emotional and physiologic response continuously during infusion.
➢ Document the patient’s action.
➢ Encourage range-of-motion exercises.
➢ Ensure that someone stays with the patient during the physical therapy.
