appendicitis. During the morning nursing rounds, the patient reported experiencing pain at the surgical site.
The nurse will assess and implement interventions for pain management.
Nurse Nayla : “Assalammu’alaikum. Good morning, Mr. Ahmed. (Performing introduction and
asking patient’s condition) “
Mr. Ahmed : “Wa’alaikumsalam. Morning, Nurse. I've been having a lot of pain in my lower
abdomen since yesterday. “
Nurse Nayla : (Responding to the answer and asking about the quality of the pain)
Mr. Ahmed : “I’m feeling sore. Moving around really hurts. “
Nurse Nayla : (Showing empathy and asking about the area of the pain)
Mr. Ahmed : “I feel the worst pain especially around the lower right side of my abdomen. “
Nurse Nayla : (Validating the feeling and asking about the severity of the pain)
Mr. Ahmed : “I would rather say that it is around 7 especially when I try to move. “
Nurse Nayla : “Thank you for letting me know. I can see that the discomfort is affecting you. You’ve
been through a lot, and it’s okay to take it easy. “
Mr. Ahmed : “I am trying to cope with this pain but it feels really uncomfortable. “
Nurse Nayla : (Showing empathy and introducing as well as suggesting a relaxation method that is
warm compress to the patient)
Mr. Ahmed : “Sure, Nurse. I will be willing to try anything that may reduce this pain. “
Nurse Nayla : “Great. I am now preparing the warm compress for you, Sir. Now, I am placing it gently
on the area near the pain, not on the incision itself. It will make your muscles relaxed
and reduce the pain. How do you feel now? “
Mr. Ahmed : “Alright, Nurse. I feel much better even though the pain remains. “
Nurse Nayla : (Showing appreciation and encouraging the patient)
Mr. Ahmed : “Thank you, Nurse. I really appreciate your help. “
Nurse Nayla : (Saying that you are leaving as well as offering help when needed)
Mr. Ahmed : “See you, Nurse. Wa’alaikumsalam. “
