I. POLICY: ADVANCED DIRECTIVES

Hopess will determine the resident’s legal guardian or representatives existing knowledge of
Advance Directives and provide the individual(s) with information on this issue. All resident’s
will be resuscitated, provided with emergency care, and transported to the nearest medical center
should a medical emergency arise.

II. PURPOSE:
To delineate the process of determining the resident information and provide education on the
Hopess Policy on Advance Directives.

III. PROCEDURE:

A. Upon admission:

1. Each resident’s legal guardian or representative will receive written information
concerning their individual rights under Arizona law to make decisions concerning medical care.

2. The resident’s legal guardian or representative will review and sign a statement of policy
on Advance Directives on the Resident Information Sheet, attesting to the knowledge of agency
policy.

B. The resident will receive care regardless of whether their legal guardian or representative
has executed Advance Directives on their behalf.

C. If Advance Directives have been executed, a copy will become part of the resident’s
permanent record.

D. Initial education for all Hopess employees will be provided during their orientation.
Subsequent training sessions will occur as changes in forms or statutes arise.
