Work Order and Job Site Preparation Example cont.
Tom made a timely logbook entry regarding the WAS Pump 1 failure and proceeded to the
field and did the following:
1. Tom first checked the WAS Pump 1 main breaker, seeing that the breaker was ON and
not tripped.
2. Tom proceeded to WAS Pump 1 checking to ensure the local Pump OFF/ON switch was
turned ON
3. Tom checked to ensure the WAS Pump 1 valving was correct.
4. Tom contacted his Lead Operator and coordinated locking out WAS Pump 1.
5. Once locked out, Tom attempted to turn WAS Pump 1 shaft.
(WAS Pump 1 is a centrifugal pump, so the shaft should turn easily but, in this case, did
not)
6. Tom removed the pump inspection plate finding a ball of rags and removed them.
7. Tom replaced the inspection plate, removed the system locks and tested the pump.
