6. Safety Sytems
1. Gas detectors : Update list of Gas detectors is not available. At present
based on OMS request and notification, the defective GDs are.
2. DCS Alarms : All DCS alarms were kept in silent mode. To be kept on
line after attending unwanted / defective alarms.
3. Cleaning pigging of 42” crude line is not done since line commis-
sioned.
4. Control room to be shifted to a safe place as per OISD recommenda-
tion.
5. ISA, ESA,OISD audit recommendations : Not liquidated completely.
6. Mountable bullets header : LPG, Kothari, cetex sampling points are
covered with pipelines.
7. Safety showers are not available near pump houses.
8. Available safety showers at Fire training ground, OMS-III and ETPs are
not working.
9. Telephones are not working.
10. Hi, HiHi and SFH alarms are not there as per recommendations given in
OISD. Hi, HiHi and SFH alarms.
11. SOPs for Tank changeover is not there and SOP is not available with
SIC of OMS.
12. Tank Level Instruments calibration is not available.
