              A confidential file will be maintained on each team member containing all employment related
              documents  such  as  the  employment  application,  resume,  job  and  salary  history,  health
              assessment  records,  documentation  of  ongoing  professional  development,  performance
              appraisals, disciplinary action, general correspondence and other documents that pertain to
              employment with Jack & Jill Children’s Center. To comply with the Americans with Disabilities
              Act,  Jack  &  Jill  Children’s  Center  keeps  all  medically  related  information  in  a  separate
              confidential file.

              Any false statements made by team members on their employment applications or personnel
              records will result in disciplinary action up to and including termination.

              Requests by team members to see their file will be considered. Personnel files may not be
              removed from Jack & Jill Children’s Center Halmos Family Building 1315 West Broward Blvd,
              Fort Lauderdale, FL 33312. Team members are prohibited from removing any documents from
              their personnel file.

              Team member requests for copies of the documents contained in the personnel file must be
              made in writing to the Human Resources Manager. Copies of Personnel Records will only be
              released to the team member and/or their attorney at law.  Attorneys at Law must present a
              Letter of Representation in order to receive a Personnel Record.

              Violation of this policy will result in disciplinary action up to and including termination.
