Methods of Payment

The plan/policy must have at least one method of
payment/reimbursement that provides the dentist with 100% of the

contacted amount and does not require the dentist to incur a fee to

receive payment

The plan/policy must disclose on its website any fees associated with

available methods of payment or reimbursement on request of the dentist

or the beneficiary
