Exception
A plan or policy does not have to comply if the policy/plan provides
payment as:
• An indemnity benefit based on a fixed schedule,
• On a cash-payment only basis,
• Directly to the beneficiary, and
• Does not provide for a copayment, deductible, a network or
contracting provider
