Initially, extensive surgical debridement and osteosynthesis will be performed. Given the
vascular injury affecting the digits, a saphenous vein graft will be harvested, with proximal
anastomosis performed on the ulnar artery and distal end-to-side anastomoses onto the
intermetacarpal arteries at the various web spaces.
Successful revascularization can thus be achieved, allowing continuation of the
reconstruction and ensuring appropriate skin coverage. Note the associated median nerve
injury, visible as a transection at the level of the palm.
