WHAT WAS THE PROBLEM?

Delayed in supplying blood for patient in acute bleeding event intraoperatively especially O&G patient since staff need to get the BHT/formal details of patient in OT
before going to blood bank to collect blood or blood components.

WHAT WAS CHANGED, IMPROVED OR IMPLEMENTED?

Implementation of blood collection slip so that staff only need to fill the patients detail in the form without having to get the patient’s BHT to collect blood and blood
components.

BEFORE AFTER

1. Staff collect blood and blood components by bringing the proof of Using the blood collection slip,
patient’s detail in form of BHT, Identity card images, registration sticker.
staff only have to fill in the slip
2. If patient in the OT and need for blood and blood component
transfusion, staff need to get the patients detail in OT before proceed to accordingly and go to blood bank
blood bank. to collect blood and blood components.

3. Hence, there was delayed in supplying blood especially in acute
bleeding.

WHAT WAS THE BENEFITS?

1. Time saving especially in acute bleeding events.

2. A safer way as staff don’t need to bring over patient’s BHT and IC images as it is confidential.

3. Avoid mislocated documents.
