                    WHAT WAS THE PROBLEM?

                    Delayed in supplying blood for patient in acute bleeding event intraoperatively especially O&G patient since staff need to get the BHT/formal details of patient in OT
                    before going to blood bank to collect blood or blood components.





                    WHAT WAS CHANGED, IMPROVED OR IMPLEMENTED?

                    Implementation of blood collection slip so that staff only need to fill the patients detail in the form without having to get the patient’s BHT to collect blood and blood
                    components.
