WHAT WAS THE PROBLEM ?
1. Pathology staff has to register the patient’s demographic data and requested test before proceed to analytical process. The process include
checking the request form for completeness as well as ensure specimen was send according to criteria, and attach barcode sticker to the
specimen and request form.
2. Clinician need to fill in the request form and specimen label manually.
WHAT WAS CHANGED, IMPROVED OR IMPLEMENTED?
- Demographic data and request of test was done by the requesting clinician in ward.
- Clinician will print out the PER-PAT form and barcode number from the LIS system and attach the barcode sticker which contains the patient
details and name of test onto the specimen.
- Lab staff has to check in the sample by scanning the barcode number that has been attached on the specimen which indicate the samples
has been received by lab.

WHAT WAS THE BENEFITS?
1. Reduce timing for registering process of the specimen.
2. Less hassle and time saving for clinician as they do not have to write the specimen label and request form manually
3. Reduce typing error.
