WHAT WAS THE PROBLEM?
1. Waste of paper due to usage of request form in routine tests
WHAT WAS CHANGED, IMPROVED OR IMPLEMENTED?
All routine test doesn’t have to send the request form. Clinician only have to register the patient’s demography data and test request
through the LIS system and attach the specimen label with printed barcode on the specimen.

WHAT WAS THE BENEFITS?
1. Time saving as lab staff does not have to attach the barcode specimen onto the request form.
2. Space saving as lab does not have to keep the request form
