WHAT WAS THE PROBLEM?
Screening of request forms for outsource specimen (special test) was done without any checklist items. Hence there was risk of rejection by the
referral lab if the required items/documents was not fulfilled.
WHAT WAS CHANGED, IMPROVED OR IMPLEMENTED?
A checklist form for special test was created to help the MLT and Medical officer screen the form and check the details needed to be fulfilled for
the requested test. It includes the location of the referral lab, criteria of the samples needed, and criteria for the test itself as well as supporting
documents needed.

WHAT WAS THE BENEFITS?
1. Prevent any missing details.
2. Serve as guideline to the screener on what criteria need to be fulfilled before sending the test to referral lab.
3. Reduce risk of rejection by the referral lab when the screener fulfilled the checklist criteria.
4. Therefore reduce sample rejection rate.
