WHAT WAS THE PROBLEM?
Borang Penghantaran Spesimen Makmal was filled in manually by Porter/PPK in charge which resulted in risk of spelling error and poor
handwriting.
WHAT WAS CHANGED, IMPROVED OR IMPLEMENTED?
Clinician will print the “Borang Penghantaran Spesimen” that was generated upon requesting the test from the LIS system.

WHAT WAS THE BENEFITS?
1. Time saving as porter/PPK doesn’t have to write the list of the tests.
2. Eliminate the spelling error.
3. Easy for Lab staff to counter check the list and specimens
